Please use this Google form to enter your FULL order which is all of your individual orders complied into one big order by end of the day Monday, November 21, 2022.

(Enter the total order for your athlete - row 27 in the Excel spreadsheet - not the individual orders of your purchasers.)

• $20 & $50 Gift Cards (Athlete profit is 15% of GC value)

Pick-Up at Lacoste Garden Centre on Sun, Dec 4 from 1-3pm

Where: Lacoste Garden Centre (2787 St. Mary's Rd)      

When: Sunday, December 4, 2022 from 1-3pm.

• Each family will be responsible for picking up their order at Lacoste for this fundraiser.     

• Please see the cashier in the store, state that you are picking up your fundraising order with Vortex Water Polo and you will be directed to the pick-up point.

• Please bring your completed Order Form with you to ensure that you receive your full order.

• Vortex's Fundraising and Merchandising Coordinator, Erin Bueckert, will be at the centre from 1-3pm on December 4 to assist with the pick-up.

Notes

• KEEP your "Athlete's Order Form" form for the distribution of the items to your purchasers.

• ENTER your FULL order into the Google form by end of the day Monday, November 21, 2022.

• SEND an e-transfer with your FULL total owing to fundvortex@gmail.com by end of day Monday, November 21, 2022.

• BE AVAILABLE to go to the Lacoste Garden Centre (2787 St Mary's Rd) between 1-3pm on Sunday, December 4 to pick-up your order.
